Federally Qualified Health Centers

One Operating Layer for FQHC Care Operations

FQHCs operate across clinical care, quality measurement, care management, reimbursement, compliance, reporting, and community-health requirements. VitalEdge connects those workflows through shared clinical evidence, encounter context, care operations, quality, and compliance infrastructure.

VitalEdge is not an HRSA reporting system and does not replace your EHR — it is the layer that keeps evidence, workflows, and requirements connected across the systems you already run.

The Operational Reality of Community Health

Community health centers carry a heavier operational load than a typical practice: UDS and other reporting obligations, sliding-scale and eligibility documentation, care-management workload for populations with real barriers to consistent follow-up, and quality requirements layered on top of already-thin administrative staffing. None of that changes what the clinical work itself looks like — a visit, a device reading, an assessment, a care-team note — but it does mean that evidence has to answer to more downstream requirements than it does elsewhere.

The result in most health centers isn't a shortage of data — vitals get logged, assessments get completed, encounters get documented. What's often missing is the connective layer between that evidence and the requirement it's meant to satisfy, so a quality measure gets reconstructed by hand from records that already contain the answer, and a reporting deadline becomes a data-gathering project instead of a byproduct of care already delivered.

The challenge is not a lack of data. It is keeping evidence, workflows, requirements, and actions connected.

How the Layers Connect

VitalEdge is built to connect these layers — from what's required, to the evidence that satisfies it, to the care action it drives, to what your team can show on review.

Not every HRSA or UDS requirement is automated end to end today — this is the architecture the platform is built around, and where connective work continues to be added.

What's Actually in the Platform

Care Programs

  • RPM
  • RTM
  • CCM
  • PCM
  • BHI

Clinical Operations

  • Triage
  • Patient context
  • Care gaps
  • Assessments
  • Care pathways

Quality

  • Care-gap visibility
  • Quality measure tracking
  • Population-level operational visibility

Compliance

  • Audit trail
  • Consent management
  • Access logging
  • Retention & legal holds
  • Evidence packs

Reimbursement Readiness

  • Program eligibility
  • Billing readiness
  • Time tracking
  • Documentation prerequisites

Interoperability

  • FHIR R4
  • Device connectivity
  • APIs
  • Clearinghouse / authorization infrastructure

FHIR R4 supports read/write for Patient, Observation, and MedicationStatement, with read access to Condition, Practitioner, and Medication. Device and clearinghouse adapters (Omron, Tenovi, Availity, Change Healthcare) are code-complete and sandbox-tested, ready to activate with vendor and payer credentials.

Connect Care Delivery to Quality Reporting

VitalEdge's quality-measure engine tracks performance aligned to selected HEDIS, MIPS, and CMS Star Rating methodologies — custom, curated rule logic, not certified eCQM or submission-ready reporting. It includes health-equity disparity stratification by race, ethnicity, and language, aligned to UDS Table 7.

The goal is to help connect the underlying evidence and workflows used for quality tracking and reporting — not to automate UDS submission or replace your reporting system of record.

Built for Reviewable Care

What the organization reports should be traceable back to the underlying evidence and care activity behind it. VitalEdge is built around HIPAA's technical safeguards from the start — not a claim of universal regulatory compliance, but a foundation of encryption, access control, and audit logging that a reviewer can actually follow.

  • Evidence provenance back to the encounter and source record
  • Comprehensive audit logging of who accessed or changed data, and when
  • Patient-facing consent management, revocable and purpose-specific
  • Patient access logs and role-based access control
  • Retention and legal holds that never purge below the HIPAA floor
  • One-click evidence packs assembled for payer, auditor, or accreditation review

What This Is Designed to Change

Operational outcomes the architecture is built to support — not a projected dollar figure.

Fewer disconnected workflows across care, quality, and billing
Greater visibility into care gaps before they become compliance issues
Better evidence traceability from result back to source record
Less manual reconciliation between clinical and billing data
Standardized care-program execution across your care team
More consistent documentation, less variation by clinician
Clearer operational oversight for leadership and quality staff
